چکیده

Malignant mesothelioma is a very serious disease. The contemporary treatment in operable cases involves a combination of neoadjuvant chemotherapy, extrapleural pneumonectomy, and postoperative radical radiation therapy. In these patients, intensity modulated radiation therapy (IMRT) techniques are used to achieve a better therapeutic ratio [1–5]. However, most cases are inoperable due to various reasons and require radical or palliative radiation therapy. In this study, a newer radiotherapy technique, volumetric modulated arc therapy (VMAT), was evaluated for the treatment of inoperable malignant pleural mesothelioma (MPM) patients. We compared the impact of two different multileaf collimator (MLC) widths on VMAT plans for MPM. All VMAT plans were generated for two Elekta commercial MLC devices using the Monaco treatment planning system (version 5.0, Elekta AB, Sweden).

Proton Therapy for Malignant Pleural Mesothelioma: A Three Case Series Describing the Clinical and Dosimetric Advantages of Proton-Based Therapy

Malignant pleural mesothelioma (MPM) is a malignancy of the pleural cavity that typically presents at an advanced stage. Due to its large, circumferential clinical target volume (CTV) and proximity to major structures, including the heart and contralateral lung, delivering hemithoracic intensity-modulated radiotherapy (IMRT) with photon therapy to achieve loco-regional control following macrosc...

A pilot study of volumetric‐modulated arc therapy for malignant pleural mesothelioma

Malignant pleural mesothelioma (MPM) is an extremely difficult disease to treat. This pilot study investigates the feasibility of using volumetric-modulated arc therapy (VMAT) for malignant pleural mesothelioma (MPM), and compares VMAT to static field intensity-modulated radiotherapy (IMRT) for five patients. To identify the best treatment technique for MPM, in five patients, we made a represen...

Intensity-modulated radiation therapy with simultaneous integrated boost in unresected left-sided pleural mesothelioma: a case report.

A 77-year-old male patient with unresected malignant pleural mesothelioma, clinical stage T3N0M0 according to the New International Staging System for Diffuse Malignant Pleural Mesothelioma, received intensity-modulated radiotherapy (IMRT) with a simultaneous integrated boost (SIB) after 6 cycles of chemotherapy with cisplatin and pemetrexed. SIB-IMRT delivered 40.5 Gy (1.5 Gy/fraction) to the ...

Comparison of Volumetric Modulated Arc Therapy and Three-Dimensional Conformal Radiotherapy in Postoperative High-Grade Glioma: A Dosimetric Comparison

Introduction: We aimed to dosimetrically compare three-dimensional conformal radiotherapy (3D-CRT) and volumetric modulated arc therapy (VMAT) in terms of planning target volume (PTV) coverage, organ at risk (OAR) sparing, and conformity index (CI). Material and Methods: Planning data of 26 high grade glioma (HGG) patients were used. Prescrib...

Functional image guided radiation therapy planning in volumetric modulated arc therapy for patients with malignant pleural mesothelioma

PURPOSE To investigate the incorporation of functional lung image-derived low-attenuation area (LAA) based on 4-dimensional computed tomography (4D-CT) in volumetric modulated arc therapy (VMAT) planning for patients with malignant pleural mesothelioma (MPM) after extrapleural pneumonectomy. METHODS AND MATERIALS Twelve patients with MPM after extrapleural pneumonectomy were included. The pri...
